YOUR DOT PHYSICAL EXAM

We make it SO EASY.  Certification is possible in less than 45 minutes!

Only $79 (cash, credit/debit/check).

  1. DOWNLOAD YOUR PHYSICAL EXAM PAPERWORK

  2. SCHEDULE and Complete your examination with DR. ERIK SCHMIDT (Certified Medical Examiner)

  3. RECEIVE your DOT Medical Card to KEEP WITH YOU AT ALL TIMES WHILE DRIVING

(2) copies of your physical exam form will also be given to you (one to keep with your CDL, and one to provide to the company you work for, if needed).  A digital copy of your physical exam form and medical card will be kept on file at our office.

Our goal is to keep you on the road!

We know failing your physical can lead to lost time at work, and that is the last thing we want to happen.

Advice and Tips for your upcoming DOT physical exam:

  1. Are you a smoker? We STRONGLY suggest that you stop smoking prior this exam (specialized testing is required of all those >35 YOA who identify as current smokers, delaying your certification significantly) and document that you quit smoking on your history form.

  2. Worried about your blood pressure?

    1. Make sure you take your blood pressure medication.

    2. Cut back on salty foods, nicotine, caffeine (coffee and energy drinks)

BEING PREPARED FOR YOUR EXAM WILL MAKE IT “HASSLE FREE”:

  1. Do not forget your glasses and/or hearing aids if you use them.

  2. Drink a glass of water before you leave (dehydration can alter your urinalysis).

  3. Bring any previous waivers or exemptions.

  4. Bring any documentation from your PCP regarding any existing health conditions*(see below).

  5. Bring the contact information of your doctors in case we need additional information.

  6. Wait to use the restroom.  You are required to give a urine sample.

*DIABETIC: Recent HbAIC and fasting blood sugar blood readings for NID diabetics

*SLEEP APNEA: CPAP users must have a digital printout of usage OR a note from lung specialist regarding compliance with the CPAP.

Bring the exam paperwork you printed and already filled out, your photo ID (driver’s license), and a list of your current medications.

What to expect during your DOT exam:

  1. Review of Health History

  2. Height, weight, BP, pulse rate

  3. Vision and hearing

  4. Heart and Lung evaluation

  5. Joint and muscle testing

  6. Basic Urinalysis (protein, sugar, blood)
